Yes, I would like to sign up for a DreamTrips membership which will be paid through payroll deduction. DreamTrips Membership: $19.95/month - You save the one-time initiation fee of $199 with corporate sponsorship. Most members save their membership cost (between you and a quest) within the first two trips. We'll offer at least 50 WorldVentures DreamTrips per year. Dependent children may go on DreamTrips by paying the trip charge and/or an additional Dependent Campanion Fee (DCF). For details see the Terms and Conditions.
I have read and understand the Terms & Conditions as presented. I affirm that none of the spaces intended to be filled out by me have been left blank. I am aware that I am entitled to a copy of this agreement at the time I sign it. I may cancel this transaction at any time prior to midnight of the third business day after the date of this transaction (5th business day if I reside in AK; 30th day if I reside in KY; 5th day if I reside in WI). I authorize my employer to payroll deduct from my pay $19.95 per month divided by the number of times I am paid per month. This authorization is to remain in full force and effect unless I provide written notification to my employer within 5 business days prior to the first of the month. I understand that if I leave my current employer I have the right to continue my DREAMTRIP membership and only need to notify AAP, Inc. to change my billing process. I certify that no such representations have been made to me by WorldVentures or any independent representative.
